Observations 

You will be prompted to create and Observation if an Inspection item is marked as X or Fail

In the pop up Observation page enter the following:

• Select the relevant type

• Leave the status as Initiated 
• Title: as this is created from a fail item in an Inspection this will auto-fill and you will not need to change it
• Assignee: Cook Brothers Site Manager or Leading Hand 
• Distribution: CBC Site Manager, CBC Project Manager, and CBC Leading Hand
• Enter the relevant Contributing Condition and Behavior
• Select the relevant Hazard type
• Description: summary of the hazard 
• Attach any photos of the hazard 
• Create
