Observations are generally created for Inspection items that require actioning or for any actionable notes during a site walkthrough.

- Select the relevant type
Leave the status as Initiated
Title: a few words to describe the item
Assignee: Cook Brothers Site Manager or Leading Hand
Distribution: Cook Brothers Site Manager, Cook Brothers Project Manager, and Cook Brothers Leading Hand
Enter the relevant Contributing Condition and Behavior
Select the relevant Hazard type
Description: summary of the hazard
Attach any photos of the hazard/item
Create
